Niacin (vitamin B3) at prescription doses hits a receptor called GPR109A on fat cells.

That slows the release of free fatty acids into the blood. With less raw material reaching the liver, the liver makes less VLDL, which means lower triglycerides and lower LDL — and HDL goes up. The "extended release" part spreads the dose out so the flush is smaller and the liver gets a steadier, lower peak than instant-release niacin.
